To complete your appointment request information,
please upload pictures of the front and back of your insurance card.

Here’s how:

  1. Create an Account: TheraNest Client Portal Software - Sign Up

  2. Log in to your portal: Ensora Health Client Portal Software - Log in

  3. Go to Documents

  4. Click Upload Files

  5. Select and label your insurance card images (front/back)

  6. Submit and confirm the upload
